After a 50-Hour Face Transplant, He Calls Her New Appearance “the Greatest Gift

For six years, Derek, 58, went through extensive facial reconstruction surgeries, yet the outcome left him unrecognizable. He lost an eye, struggled to breathe and speak, and depended on a feeding tube. Despite everything, he remained thankful for the care he received.

When all traditional surgical options were exhausted, his medical team proposed a face transplant — his final hope. The operation, conducted at the Mayo Clinic, became their most complex to date, replacing 85% of his face with a donor’s forehead, eyelids, nose, mouth, and jaws. Surgeons meticulously reconnected delicate nerves to restore essential functions such as blinking, eating, and even smiling.

After waiting nine months, a donor match was found, and in February the transplant was carried out over two and a half days. One month later, Derek saw his new face for the first time — a deeply emotional moment. His mother, Lisa, recalled that it was the first time in years that people no longer stared at him in public.

Now able to speak and smile again, Derek is rebuilding his life as a motivational speaker, using his experience to raise awareness about mental health and suicide prevention. He and Lisa have helped families reconnect and have even contributed to saving lives through their advocacy.

Looking forward, Derek hopes to find love, get married, and start a family — dreams made possible by an extraordinary gift from a donor’s family.
